An APCP Fellowship Award Nomination Form must be completed by 2 individuals:
Primary Nominator - this person must be a current member of APCP
Secondary Nominator - this person does not need to be a current APCP member
We recommend that you do not tell the nominee about the nomination, so as to avoid disappointment should the nomination be unsuccessful.
Nomination Form
The form should be completed jointly by the 2 nominators and will include:
- details of the nominee and nominators (and their relationship to the nominee)
- a brief testimonial demonstrating why and evidencing how they think the individual has made an exceptional contribution to the profession.
